About the Role:
We are seeking a highly skilled Nurse Practitioner to join our team of compassionate care specialists. In this role, you will provide expert palliative care to patients with serious illnesses, focusing on alleviating burdensome symptoms and aligning medical treatments with their goals and values.
You will work closely with a multidisciplinary team to deliver high-quality patient care, wherever they call home. As an active member of the medical community in your territory, you will provide ongoing palliative care and hospice services, as well as education and support to patients and families.
Responsibilities:
- Assess and manage complex patient needs, working collaboratively with other healthcare professionals
- Develop and implement personalized care plans, taking into account patients' goals, values, and preferences
- Provide symptom management and pain control, using evidence-based practices and medications
- Counsel patients and families on end-of-life care options, advance directives, and other relevant issues
Qualifications:
- Bachelor's degree in Nursing or related field
- Current licensure as a Registered Nurse (RN) and certification as a Nurse Practitioner (NP)
- Minimum 2 years of experience in chronic disease management or long-term care, with a focus on palliative care
- Excellent communication and interpersonal skills, with ability to work effectively with diverse patient populations
