Job Description
">You will provide patients with diagnosis, treatment, and care under the supervision of a physician. Your focus will be on providing palliative care, assisting families along their health care journeys with love and compassion.
Required Qualifications
">- A heart to serve patients and families and a passion for providing the best possible care
- Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing
- 2 years experience within the last 5 years in chronic disease management/long-term care. Hospice background preferred
- Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 25lbs
