**Job Responsibilities:**
Provide ongoing palliative care and hospice services to patients in their homes
Educate patients and their families on how to manage their symptoms and improve their quality of life
Collaborate with other healthcare professionals to deliver comprehensive care
Work closely with the patient's primary care physician to ensure seamless care transitions

**Required Qualifications:**
A heart to serve patients and families and a passion for providing the best possible care
Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing preferred
2 years experience within the last 5 years in chronic disease management/long-term care. Hospice background preferred
Ability to sit, stand, bend, move intermittently and lift at least 25 lbs and bear the weight of an average adult effectively
