Qualifications :

  • A heart to serve patients and families, with a passion for delivering the best possible care.
  • Education : Graduate of an accredited master's, post-master's, or doctoral nursing program with formal preparation as a Nurse Practitioner.
  • Licensure : Current unencumbered licensure as an Advanced Practice Registered Nurse with designation as a Nurse Practitioner. Must maintain current prescriptive authority with full DEA privileges and national certification in the Nurse Practitioner specialty.
  • Experience : Minimum of 2 years of experience within the last 5 years in chronic disease management or long-term care. Hospice or Palliative experience is preferred.
  • Required : Reliable transportation. Ability to sit, stand, bend, move intermittently, and lift at least 25lbs.
