In this role, you will provide care to seriously ill patients, focusing on alleviating distressing symptoms and aligning treatment with patients' goals and values. Collaboration is key, as you will work with a team of like-minded healthcare professionals to ensure the highest level of care is provided to patients wherever they reside. As a vital member of the medical community in your area, you will also provide ongoing palliative care and hospice services, along with educational resources.

Qualifications :

  • A heart to serve patients and families, with a passion for outstanding care
  • Education : Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing
  • Experience : 2 years of experience within the last 5 years in chronic disease management / long-term care. Hospice experience preferred.
  • Required : Reliable transportation; ability to sit, stand, bend, move intermittently, and lift at least 80-100 lbs.
