Key Responsibilities:
- Conduct thorough assessments of patients' physical, emotional, and spiritual needs
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams
- Provide education and support to patients and families on disease management and end-of-life care
- Maintain accurate and timely documentation of patient interactions and care provided
Requirements:
- Master's degree in Nursing from an accredited institution
- Current licensure as a Registered Nurse (RN) and Advanced Practice Registered Nurse (APRN)
- 2 years of experience in chronic disease management or long-term care, with a preference for hospice background
- Reliable transportation and ability to lift at least 80-100 pounds
