Job Responsibilities
In this role, you will be responsible for conducting patient visits, providing routine care, and communicating with the care team, patients, and their families. You will also have the opportunity to use your skills to make a real difference in the lives of those we serve.
Required Skills and Qualifications
- A heart to serve patients and families
- Graduate of an accredited school of nursing with a current state license as a licensed practical nurse
- 2 years of nursing experience in an acute care setting, or equivalent experience (hospice experience preferred)
