About the Role
We are seeking an experienced Medical Care Specialist to join our team as a Hospice and Palliative Care Nurse Practitioner. As a key member of our clinical staff, you will work closely with patients and families to provide high-quality care and support.
Job Summary
- Deliver palliative care services to patients in their homes, hospitals, or other healthcare settings.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Conduct thorough assessments of patient needs and preferences.

Requirements
- Master's degree in Nursing or related field.
- Current licensure as a Nurse Practitioner (NP) or Physician Assistant (PA).
