Hospice and Palliative Care Nurse Practitioner

We are seeking a compassionate and skilled Hospice and Palliative Care Nurse Practitioner to join our team. As a member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals with serious illnesses.

Job Summary

The ideal candidate will possess a strong passion for delivering palliative and hospice care in a variety of settings, including patients' homes. You will work collaboratively with a team of healthcare professionals to develop and implement individualized care plans that address the physical, emotional, and spiritual needs of our patients.

Responsibilities:

• Provide comprehensive palliative and hospice care to patients in their homes or other settings.
• Conduct thorough assessments of patients' medical, emotional, and spiritual needs.
• Develop and implement individualized care plans that address patients' goals and values.
• Collaborate with healthcare teams to ensure seamless care coordination.
• Document patient information accurately and thoroughly.

Requirements:

• Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing.
• At least 2 years of experience in chronic disease management/long-term care within the last 5 years.
• Hospice background preferred.
• Ability to sit, stand, bend, move intermittently, and lift at least 25 lbs.

Salary: $120,000 - $180,000 per year
