- Evaluating patients and creating individualized care plans that meet their unique needs.
- Communicating effectively with patients, families, and the care team to ensure that everyone is informed and involved in the care process.
- Collaborating with other healthcare professionals to ensure comprehensive care.
- Providing emotional support and guidance to patients and their families.
Requirements:
To be successful in this role, you will need to have:
- A current state license as a registered nurse.
- 3 years of experience as a nurse in a clinical care setting or home health environment.
- 1 year of pediatric experience.
- Reliable transportation.
- The ability to sit, stand, bend, move intermittently, and lift at least 80-100 lbs.
