At Agape Care Group, we're seeking a compassionate and dedicated Nurse Care Specialist to join our team. In this role, you'll provide exceptional care and support to patients facing life-limiting illnesses during weeknights. As a key member of our hospice care team, you'll work collaboratively with healthcare professionals to ensure our patients receive the highest quality care.
About the Role:
- This is a salaried full-time position covering on-call hours from 5 pm - 8 am Monday through Thursday, ending on Friday morning at 8 am.
- You'll be fully supported by our 24/7 RN staffed Triage Department and local hospice team.
Responsibilities:
- Advocate for patients and their families, ensuring they receive quality care.
- Collaborate with the hospice care team to develop and implement patient care plans.
- Provide education and support to patients and their families, addressing their unique needs and concerns.
Qualifications:
- A graduate degree in nursing from an accredited school with a current state license as a registered nurse.
- At least 2 years of experience in a clinical care setting, preferably in hospice care.
- Ability to sit, stand, bend, move intermittently, and lift at least 80-100 pounds effectively.
