Responsibilities:
- Evaluate patients and create comprehensive care plans
- Communicate effectively with patients, families, and the healthcare team
- Lead the care team to ensure quality care for all patients
Requirements:
- Graduate of an accredited school of nursing with a current state license as a registered nurse
- 2 years of nursing experience in a clinical care setting (hospice experience preferred)
- Reliable transportation
- Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs and bear the weight of an average adult effectively
