As a hospice and palliative care nurse practitioner at Agape Care, you'll provide patients with diagnosis, treatment, and care under the supervision of a physician. In this role, you'll focus on providing palliative care. Most importantly, you'll assist families along their health care journeys with love and compassion.

Key Qualifications
- A heart to serve patients and families and a passion for providing the best possible care
- Graduate of an accredited masters, post masters, or doctoral nursing program with formal preparation as a Nurse Practitioner
- Current unencumbered licensure as an Advanced Practice Registered Nurse with designation as a Nurse Practitioner. Current prescriptive authority with full DEA privileges
- 2 years experience within the last 5 years in chronic disease management/long-term care. Hospice and/or Palliative background preferred
- Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 25lbs
