Responsibilities and Requirements
- Provide routine care and conduct patient visits

- Communicate effectively with patients and their families
- Graduate of an accredited school of nursing with a current state license as a licensed practical nurse
- At least 2 years of nursing experience in an acute care setting or equivalent experience (hospice experience preferred)
- Reliable transportation and ability to lift up to 25 lbs
