Patient Care Responsibilities:
- Provide comprehensive palliative and hospice care to patients in their homes or other settings.
- Conduct thorough assessments of patients' medical, emotional, and spiritual needs.
- Develop and implement individualized care plans that address patients' goals and values.
- Collaborate with healthcare teams to ensure seamless care coordination.
- Document patient information accurately and thoroughly.
Qualifications and Requirements:
- Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing.
- At least 2 years of experience in chronic disease management/long-term care within the last 5 years.
- Hospice background preferred.
- Ability to sit, stand, bend, move intermittently, and lift at least 25 lbs.
