Join Our Team as a Patient Care Relations Coordinator

Are you a compassionate individual with strong organizational and interpersonal skills? Do you have a passion for ensuring that patients receive the care they deserve? We are seeking a dedicated Patient Care Relations Coordinator to support our hospice patients and their families with love and integrity.

In this role, you will coordinate referrals and maintain effective communication between clinical supervisors and our agency. Your key responsibility will be to facilitate a seamless referral process, ensuring timely access to the critical care our patients need.

Qualifications

  • A genuine heart to serve patients and families, with a passion for providing excellent care
  • Education : Associate degree preferred
  • Experience : Minimum of 1-year experience in a health care setting. Preferred experience in customer service and office management
  • Requirements : Reliable transportation, ability to sit, stand, bend, move intermittently, and lift at least 25 lbs
