**Responsibilities and Expectations:**
Deliver high-quality, patient-centered care to patients in their homes
Collaborate with other healthcare professionals to develop and implement individualized care plans
Provide education and support to patients and their families on symptom management and end-of-life care

**Requirements:**
A heart to serve patients and families and a passion for providing the best possible care
Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing preferred
2 years experience within the last 5 years in chronic disease management/long-term care. Hospice background preferred
Ability to sit, stand, bend, move intermittently and lift at least 25 lbs and bear the weight of an average adult effectively
