JOB SUMMARY :
The Continuous Care LPN monitors patients and reports changes in their condition. Duties include administering treatment under the supervision of Registered Nurse Case Managers (RNCMs), updating patient files, and instructing families on patient care.
Schedule : Monday-Thursday 7 : 00am-7 : 00pm
Coverage area : Atlanta
MAJOR JOB FUNCTIONS :
- Promote and practice the Sage Family of Companies philosophy and mission of, “Providing the most compassionate, comprehensive, and qualified care to people with a life-limiting illness to serve them through their individual journey with dignity, respect, and integrity.”
- Provides nursing services to patients under the supervision of RNCM.
- Travels to patient locations to administer bedside care for crisis. When crisis is under control, continuous care will then be stopped.
- Assesses treatment plans and coordinates patient care with interdisciplinary health professionals.
- Measures and records vital signs, including blood pressure, heart rate, and pulse oximetry.
- Administers medication for pain or symptom management medication as required.
- Evaluates patients' responses to treatments and medications, as well as facilitates appropriate interventions when required.
- Instructs families and caregivers on patient care and addresses concerns in a sensitive manner.
- Updates and maintains patient files and medical records.
- Adheres to best practices in nursing and pursues continued professional development.
- Perform other duties and responsibilities as assigned.
