AGAPE HOSPICE
This position is a 7on/7off overnight position. Position will be home based, and expectation for position to travel to patient's residence to provide assessment and care as appropriate.
Job Description Summary
The On Call Nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience during non regular business hours and holidays. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the on call team to help meet positive patient care outcomes.
Essential Job Functions/Responsibilities
- Does work outside of normal business hours of 8:30AM – 4:30PM and holidays.
- Receives phone calls from Agape Hospice main line during non-business hours/holidays and addresses the needs of the caller to the best of their ability.
- Conducts patient visits and gathers information for the RN Case Manager.
- Performs treatments, including wound care, as directed by the RN Case Manager.
- Utilizes data gathered to determine nursing diagnoses and incorporate these diagnosis into the plan of care under the direction of the RN Case Manager.
- Updates the Interdisciplinary team Plan of Care and attend Interdisciplinary team meetings when necessary.
- Follows Interdisciplinary team Plan of Care when providing hospice services to the patient.
- Assists in educating the patient, family, and/or caregiver as needed on medication and care.
- Assists in counseling patient, family, and/or caregiver as related to healthcare needs.
- Assists in identifying discharge-planning needs.
- Reports changes in the patient’s condition or care needs to Attending and to RN Case Manager.
- Obtains and records physician’s orders as needed.
- Orders medications, DME and other supplies to be delivered to patient as needed
- Documents patient care, visit summaries and services per Agape Hospice & Palliative Care policies.
- Participates in quality assessment and performance improvement activities.
- Follows all Agape Hospice & Palliative Care policies and procedures.
- Assists in ensuring compliance with all state, federal and Joint Commission with referral/intake regulatory requirements with regard to hospice intakes.
- Works with the Admissions Director and admissions team to ensure timely evaluation and admission of patients to hospice, when the admission is scheduled outside of normal business hours.
- Ensures seamless transition of patients to hospice care by providing direct oversight of patient education and preparation for hospice care, plan of care initiation and coordination of care with multiple service providers when pt admitted outside of normal business hours.

Position Qualifications
- Current licensure to practice professional nursing in the state of Arizona.
- Graduate of National League for Nursing accredited school of nursing.
- Maintains a current CPR certification.
- Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
- Minimum of two years experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred.
- Self directed and able to work with minimal supervision.
- Demonstrates excellent observation, problem solving, verbal and written communications; nursing skills per competency checklist.
