Job Summary
Agate Integrated is a dedicated behavioral health clinic committed to serving individuals experiencing substance use disorders, homelessness, and co-occurring mental health disorders. We are seeking a passionate and driven Outreach Specialist to engage and support at-risk individuals in our community. The ideal candidate will work directly with clients, providing education, advocacy, and connection to critical services. This role requires a deep understanding of behavioral health challenges, excellent communication skills, and a commitment to making a meaningful impact.
Duties
- Conduct outreach efforts to identify and engage individuals in need of behavioral health services.
- Develop relationships with local shelters, community organizations, and service providers to expand referral networks.
- Provide education and resources on substance use disorders, mental health, and harm reduction strategies.

- Develop and execute outreach initiatives to engage the community and educate them on Agate's behavioral health services.
- Collaborate with local organizations, schools, and community groups to promote health education.
- Conduct needs assessments to identify community health issues and gaps in services.
- Lead fundraising efforts to support outreach programs and initiatives.
- Assist clients in navigating healthcare and social service systems, ensuring access to appropriate care.
- Conduct screenings and assessments to determine client needs and refer them to appropriate services.
- Work collaboratively with clinical staff, case managers, and peer support specialists to develop individualized service plans.
- Maintain accurate records of outreach activities and client interactions.
- Participate in community events, trainings, and advocacy efforts to raise awareness of behavioral health issues.
- Manage social media platforms to enhance visibility and engagement of outreach activities.
Requirements
- Strong background with conducting outreach.
- 1-2 years experience working with Behavioral Health Clinics.
- Experience working with individuals experiencing homelessness, substance use disorders, and/or mental health challenges.
- Strong interpersonal and communication skills, with the ability to build trust and rapport with diverse populations.
- Experience with networking and partnering with community stakeholders.
- Knowledge of harm reduction principles, trauma-informed care, and community resources.
- Ability to work independently and as part of a team in a fast-paced environment.
- Bilingual (Spanish/English) preferred but not required.
- Valid driver’s license and reliable transportation required.
Job Types: Part-time, Contract
Pay: $15.00 - $20.00 per hour
Expected hours: No less than 10 per week
Schedule:
- Weekends as needed
