Age in Place Home Care is looking for a reliable and compassionate Home Care Nurse (LPN/RN) to join our team. The nurse will perform routine visits to the patients’ homes to ensure quality of care. This is a flexible, part time position that is perfect for an applicant looking to create their own schedule!
Responsibilities
- Perform an initial assessment of patient's care
- Perform routine 90 care visits to reassess and reevaluate the care of patient
- Monitor patient’s condition and assess their needs to provide the best possible care and advice
- Report observations and any change in patient's symptoms or care to the agency
- Observe and interpret patient’s symptoms
- Collaborate with care team to devise individualized care plans for patients
- Inspect the facilities and act to maintain excellent hygiene and safety
- Foster a supportive and compassionate environment to care for patients and their families
Skills
- Proven experience as a LPN/RN
- Experience in home care nursing
- Excellent knowledge of nursing care methods and procedures
- In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination etc.) and willingness to follow them at all times
- A team player with excellent communication and interpersonal skills
- BSC or other diploma from a nursing program
- Valid LPN/RN license
Compensation
- Compensation is tied to admission and visitation
- RN: $25 each visit/ $35 each admission
- LPN: $20 each visit/ $25 each admission
- Estimated 10 hour compensation: $200-$400
Job Type: Part-time
Pay: $200.00 - $400.00 per day
Expected hours: 10 – 30 per week
