What are the responsibilities and job description for the Caregiver Support Specialist position at Age Well?

The Caregiver Support Specialist is responsible for providing information about resources and supportive options counseling to caregivers of older adults to promote caregiver health, independence, social and financial wellbeing. This position supports caregivers through education and coordination of services that provide respite or help to facilitate safe, effective, and compassionate caregiving. The Caregiver Support Specialist performs person-centered assessments of caregiving families to understand goals and areas of unmet need, manages the Dementia Respite Program, assuring recipients meet grant eligibility criteria and prepares and delivers community outreach presentations when needed.
*This position is 32 hours per week.
Requirements:
The successful candidate will have a College Degree with three years of related experience, experience in community organization and working with groups and possess effective communication and organizational skills.
