Case Managers assess needs and coordinate and monitor community services and benefits for people over 60 to promote health, independence, social and economic wellbeing. This is a field position of some complexity, with particular attention paid to the most needy and at-risk individuals in the community at large.
The successful candidate will have a Bachelor’s Degree and some experience is preferred.
