Nurse Practitioner Job Description
Job Summary
VAC Wound Care Specialists is hiring mid-level practitioners to treat home-bound, long term facility, ALF’s, memory care, hospice chronic wound care patients.
Responsibilities
Visiting chronic wound care patients and treating them with allografts and other wound care procedures and products. The job will also require performing basic standards of wound care as well as ancillary products and counseling for best possible outcomes for the patient.
Duties
- Conduct comprehensive patient assessments and develop individualized care plans.
- Perform medical examinations and diagnostic tests.
- Utilize aseptic techniques in all procedures to maintain patient safety and prevent infection.
- Document all patient interactions accurately in electronic health records.
- Collaborate with multidisciplinary teams to coordinate patient care and follow-up.
- Educate patients and families on health management, treatment options, and preventive care strategies.
- Monitor patients' progress and adjust treatment plans as necessary based on clinical findings.
- Collaborate with facilities on the patient's care.
Qualifications
- Master’s degree in Nursing from an accredited program.
- Current Nurse Practitioner certification and state licensure.
- Experience in chronic wound care or outpatient settings preferred.
- Proficiency in medical documentation practices.
- Knowledge of chronic wound care is a plus but not required.
- Excellent communication skills and the ability to work effectively with diverse populations.
- Strong organizational skills and attention to detail.
