What are the responsibilities and job description for the Community Case Manager position at Ageless Upstate Case Management Services?
Description - Providing Case Management for elderly and disabled clients through out the state of SC for SCDHHS Medicaid waivers. Community Choices, Vent, HIV, Hasci and ID/RD
Requirements - Hiring State Wide
Two years of case management, care planning and assessment experience.
Four year degree in a health or human services related field (Desired).
Must pass background check, submit driving record, and pass a drug test.
Must have car insurance.
Must be available from 8:30 AM - 5:00 PM Monday - Friday.
Must have reliable transportation, internet, and a WORK cell phone.
Must pass an Entry Test and complete state training as required.
Must pass 90 day competency test once training has been completed.
Must be self motivated and be able to independently manage a case load of up to 80 clients.
This job requires time management skills, good communication skills, computer skills and the ability to follow instructions per the scopes of services.
Bilingual desired but not required.
