Your Role as Health and Wellness Director :
In this role, you’ll oversee the quality, continuity, and appropriateness of care for our residents. You’ll lead and support our wellness staff, ensuring compassionate, person-centered care through effective communication with residents, families, and healthcare professionals.
Key Responsibilities :
- Lead by Example : Daily, engage with residents and staff by walking the building and staying involved in care activities.
- Care Planning : Develop and implement resident service plans per doctor orders and healthcare needs.
- Communication : Coordinate with residents, families, staff, physicians, and healthcare providers on resident health issues and care plans.
- Compliance & Review : Regularly update service plans to ensure compliance with state regulations.
- Hands-On Care : Perform and supervise individual nursing services as required.
- Staff Development : Provide leadership, training, and ongoing support for wellness staff. Conduct staff in-services as needed.
- Resident Assessment : Assess prospective residents for care needs and suitability for residency.
- Medication Management : Oversee medication setup, delivery, and storage to ensure safety and accuracy.
- Emergency Response : Respond to resident emergency calls and ensure proper documentation and follow-up.
- Risk Management : Identify at-risk residents and create comprehensive risk management plans.
- Resident Wellness : Implement programs to promote health and wellness through education and preventative care.
