Transitional Program & Housing Coordinator
Location: North Haven, CT
Schedule: Monday–Friday, 8:30 AM – 4:30 PM

About the Role:
The Transitional Program and the Housing Coordinator each play a vital role in supporting individuals with disabilities and older adults as they transition from institutional settings back into the community—or work to remain safely in their homes. You will serve as a key liaison between clients, care managers, and community partners, helping to ensure coordinated services, safe housing options, and long-term stability for those at risk.

Work Environment: In the Office and in the Field
This role includes a balance of in-office responsibilities and fieldwork. You will travel throughout the South-Central Connecticut region, working in a variety of settings including nursing homes, rehabilitation centers, and individual client homes. As part of the transition process, you'll meet clients where they are—literally and figuratively—to assess their needs, coordinate services, and ensure a smooth return or continuation of community living.
Key Responsibilities:
- Coordinate transitional and housing services for clients re-entering the community from hospitals, nursing homes, or other institutions
- Support Care Managers with service planning, resource identification, and follow-up care
- Register and track service referrals, coordinate volunteer assignments where applicable
- Conduct follow-up surveys to assess client satisfaction and program effectiveness
- Monitor compliance with program requirements and assist with reporting
- Collaborate with community partners to identify safe, affordable housing options
- Assist in planning and administering related transitional and housing support programs
- Maintain accurate records and documentation to ensure timely and appropriate service delivery
Qualifications:
- Bachelor’s degree in Social Work, Gerontology, Human Services, Public Health, or a related field
- Experience working with older adults, individuals with disabilities, and/or caregivers
- Familiarity with housing resources, Medicaid/Medicare, and community-based services is a plus
- Excellent organizational, communication, and problem-solving skills
- Ability to work independently while collaborating as part of a larger care team
- Comfortable working in diverse environments, including nursing facilities and private homes
