The Agency on Aging of South Central CT is seeking a highly motivated and compassionate Care Manager to join our team in North Haven, CT. Minimum Qualifications :
- Bachelor's degree in Human Services or related field
- 2 years of experience in case management or related field
- Knowledge of Medicaid regulations and requirements
