Position Responsibilities:
Assist Managed Care Staff with administrative and consumer related functions to support the work of the Managed Care staff, including home visits to applicants/consumers as required. Position includes home visits in the 28 cities and towns in the Merrimack Valley and North Shore.
Essential Functions: (Core duties or tasks that are fundamental and not marginal to the performance of the job)
- Assist Managed Care staff with follow-up consumer work including service suspensions/resumptions, procurement of services, accessing durable medical equipment for consumers, locating consumers who were not at home to receive services, etc. and completing consumer documentation and data entry as required.
- Assist consumers to apply for MassHealth and complete annual MassHealth renewal paperwork including making home visit to the consumer, assisting consumer to complete application/ required paperwork, obtaining, and making copies of required verifications, etc.
- Assist consumer with applying for public benefits as needed.
- Assist consumers who are requesting emergency purchases such as food or other necessary items by procuring and delivering required items to the consumer.
- Perform administrative functions to support the teams including completing standard forms (on paper or on line), data entry into member databases and A&D as well as filing, copying, etc.
- Observe all confidentiality regulations and protect consumer information.
- Attend mandatory meetings and trainings including Team Meeting, General Staff Meeting and Managed Care Staff Meeting.
- Other duties as assigned by Managed Care Program Management.
Qualifications:
Associates Degree preferred. High School diploma and a minimum of one year of related work experience required. Experience in Human Service environment helpful. Computer skills required. Must be responsible and have compassion for older adults and people with disabilities. Excellent interpersonal and communication skills a must in this team oriented position. Must have valid Driver's License and reliable transportation. A bilingual capacity in either Khmer or Spanish is strongly preferred.
Hours per week : 37.5 hours per week
