PACE Clinical Services RN Supervisor

Job Summary

Reporting to AgeWell PACE Executive Director, the PACE Clinical Services RN Supervisor is a proactive supervisor, responsible for site specific functionality, including day-to-day activities of PACE Nursing and clinical support teams. Responsibilities include ensuring direct and indirect health clinic care to PACE participants, as well as performance management of Clinic RN's, LVN's, MAs, CNAs and support clinical teams in other clinical areas as assigned. This position acts as the clinical leadership liaison between the Medical Director and clinical teams, as well as working in conjunction with the Executive Director, to ensure fluency of daily operations, and high-quality outcomes for PACE participants.

Position Responsibilities

  • Ensures and monitors direct and indirect nursing care/services to PACE participants in the Clinic and Day Center setting in a full-time and on-call capacity.
  • Responsible for the day-to-day functions and resolution of workflow disruptions within the Clinic and Day Center, including but not limited to, proactive trouble shooting, review daily clinic schedule, staffing coverage, quality assurance, quality improvement, employee performance and time sheet management.
  • Sees AgeWell PACE participants in the Clinic.
  • Coordinate services for complex client needs in Clinic and Day Center setting or delegation of complex tasks as appropriate.
  • Identifies health crises and emergency situations and independently initiates appropriate action; including ability to effectively lead various codes until completion.
  • Completion of administrative incident or accident reports as needed.
  • Clearly and effectively communicates participant health status changes to appropriate team members and coordinates or delegates participant care with day center/clinic staff.
  • Conducts routine huddles in health clinic and regular one on one meetings with each team member.
  • Ensures compliance with prescribed staffing ratios to meet patient care needs.
  • Ensures that all medications and testing materials are not expired and stored in the appropriate location, and crash carts are complete and up to date.
  • Ensures care provided conforms to accepted practice standards; provides correct treatments/procedures and other care as prescribed and according to patient care standards.
  • Ensures participant and family education is provided by the nursing team as needed.
  • Ensures all clinical quality measures are achieved, via conduction of routine audits and active participation in PACE QA processes in compliance with PACE requirements and regulations.
  • Assists with the planning and preparation of the PACE annual CMS/DHCS audit.
  • In coordination with the PACE Medical Director and the PACE Quality Manager, ensures that clinical protocols are followed, making recommendations for improvements, and instrumental in implementation of QA improvement plans.
  • Ensures a safe work environment for all team members, and immediately takes appropriate steps to resolve any risks.
  • Other duties as assigned.

  • Able to work collaboratively with physicians, nurses, ancillary clinical staff, patients, families, internal departments, and external service providers.
  • Demonstrated patient-centered care focus, advocacy, and customer service skills.
  • Knowledge of PACE and CMS requirements and regulations.
  • Advanced nursing assessment and critical thinking skills.
  • Thorough knowledge of appropriate utilization of services available to PACE participants.
  • Able to document concise yet thorough clinical documentation of patient assessment and care needs.
  • Demonstrated strong communication and customer service skills, problem solving, critical thinking, time management, organizational skills and clinical judgment abilities.
  • Technical ability to utilize Microsoft Office computer applications, EHR’s and video conferencing.
  • Complies with all department, organization, and government policies & procedures.
  • Attends meetings and trainings as required.
  • Adheres to attendance and punctuality policies and practices.
  • Performs other duties as assigned.
  • Demonstrated ability to be culturally sensitive and respect diversity, work effectively with individuals of different cultures and socio-economic status; passion for service; self-starter and highly organized; ability to prioritize and complete a large volume of work within strict deadlines; provide prompt, efficient and responsive customer service.

Experience Required

  • Minimum 5 years RN experience
  • Minimum 1 year of experience working with frail elderly populations.
  • At least 1 year of demonstrated, progressive leadership skills and responsibilities.

Preferred Experience

  • Knowledge and or experience working in senior or elder care setting - PACE, Nursing Home, Hospice, ADHC.
  • Nursing competency and familiarity with care paths and interact tools commonly used in skilled nursing home settings.
  • Must be able to triage clinical symptoms, apply care paths and clinical interactive tools in patient evaluation and care prior to communications with physicians.
  • Must be comfortable starting IV lines, IV medication administration and monitoring.
  • Must be comfortable with acute care of frail adults, complex wound care, and cognitively impaired adults.

Education Required

  • Graduate of an accredited school of nursing Bachelor’s Level Degree, or greater.

Certifications/Licenses Required

  • Valid CA RN license.
  • Valid driver’s license and reliable transportation and proof of current vehicle insurance (if applicable).
  • First Aid, CPR, and BLS certification.

Verbal and Written Skills Required to Perform the Job

  • Excellent oral, written, and interpersonal communication skills. Bilingual (Spanish/English/Arabic/Tagalog) is a plus, but not required.

Technical Knowledge and Skills Required to perform the Job

  • Possess the skills to provide high quality care for geriatric patients.
  • Must thrive in a team environment, possess good organizational and supervisory skills, and have the ability to effectively handle difficult and unusual interpersonal situations.
  • Meet a standardized set of competencies for the specific position description established by San Diego PACE and approved by CMS.
  • Knowledge of video conferencing software and Microsoft Office, including Word, Excel, and Power Point. Knowledge of Electronic Health Records (i.e. EPIC, True Chart, NextGen).

Equipment Used

  • Minor medical equipment such as vital signs monitors, emergency cart, automated external defibrillator (AED), and patient personal assist devices.
  • Networked computer with various applications, copy machines, fax machines, document center, and landline/cell phones. Personal vehicle.

§ Physical demands: Employee must be able to sit for extended periods of time. Employee will also be actively present throughout the PACE center, engaged with employees, participants and with staff. Employee is required to use hands and fingers, especially for typing on the computer and using the mouse. Employee must be able to talk and hear, particularly for daily interactions with staff and elderly participants.

§ Work Environment: Employee must be able to work in a busy and demanding clinic environment with moderate to loud noise levels. Will be required to occasionally travel to homes and facilities in Marin and Sonoma County.

Pay: $120,000.00 - $135,000.00 per year
