Job Overview
The PACE Social Worker - MSW plays a crucial role in providing comprehensive support to participants within the Program of All-Inclusive Care for the Elderly (PACE). Duties
- Conduct thorough assessments of participants' social, emotional, and environmental needs.
- Develop and implement individualized care plans in collaboration with the healthcare team.
- Provide counseling and support to participants and their families regarding care options and resources.
- Facilitate access to community resources, services, and support systems.
- Monitor participant progress and adjust care plans as necessary.
- Advocate for participants' rights and needs within the healthcare system.
- Participate in interdisciplinary team meetings to discuss participant care and progress.
- Maintain accurate documentation of assessments, interventions, and outcomes in compliance with regulatory standards.
Experience
- A Master’s degree in Social Work (MSW) from an accredited institution is required.
- Current state licensure as a Social Worker is preferred.
- Experience working with elderly populations or in geriatric care settings is highly desirable.
- Strong interpersonal skills with the ability to communicate effectively with diverse populations.
- Knowledge of community resources available for seniors and their families.
- Proficiency in case management practices and documentation standards.
- Ability to work collaboratively within a multidisciplinary team environment.
