Summary/Objective
The Personal Care Attendant/ Driver provides personal care and related services in the PACE Center and at participant’s home. Tasks to be performed must be assigned by and performed under the supervision of the PACE Attendant Care Supervisor to ensure all tasks are approved in the participant’s individual care plan and performed according to protocol.
PCA/Driver will transport participants to assigned location as directed by the PACE Attendant Care Supervisor to ensure trip is approved in the participant’s individual care plan and PCA has been trained and approved to drive company-owned vehicle. PCA/Driver will provide door-to-door escort service from both starting and ending locations, remaining on-site during appointments.
Essential Duties/Responsibilities
· Provides approved services to participants as assigned.
· Prepares a report of home visit on the day it is performed and incorporates same in the clinical record weekly as required
· Attends in-service as required by state and federal regulations and PACE training
· Assists participant in their residence with bathing, dressing, toileting, and personal hygiene following agency protocols.
· Performs routine housekeeping tasks and chore services such as laundry, dishwashing, mopping, light dusting, sweeping meeting standard of a clean and safe environment for the participant.
· May assist with home meal preparation, as assigned by PACE Nutritionist.
· Assists assigned participants while in attendance at the PACE Center including assistance with toileting, movement between activities, and feeding.
· Assists the participant with transfers from bed to chair, chair to wheelchair, etc.
and ambulation following all safety protocols each time.
· Report incidents immediately to supervisor and file incident report upon return that day to PACE.
· Assists participant in home exercises that are provided by therapy or with involvement in PACE Center activities and provides simple emotional and psychological support as trained to recognize mental illness and dementia
· Document any observed change in the participant's mental or physical condition whether observed at PACE Center, in residence, or while transporting and report it immediately to supervisor, or staff nurse, or any member of the IDT Team
· Prior to use, complete vehicle checklist and confirm that vehicle is in proper working order and clean. Update all transportation maintenance records on each trip and file with Transportation Dispatcher or Transportation Director.
· Transport PACE participants to assigned destinations in a timely and safe manner and remain present on-site during outside appointments.
· Follow established schedules and routes, with the flexibility to meet unanticipated changes, determine the safest and most efficient pick- up and delivery routes and report changes to Dispatcher.
· Assist passengers on and off the vehicle in a safe and courteous manner.
· Demonstrate understanding of handling medical emergencies as set by PACE policy and procedures; as well as the ability to work with various types of medical equipment such as wheelchairs walkers, and oxygen tanks as trained by clinical supervisor.
Required Education and Experience
· High school diploma or general education degree (GED).
· One-year experience working with the frail and/or elderly.
· Must be at least 21 years of age or older to operate company vehicles.
· Must have a valid driver's license in the state of Indiana and license must be issued for minimum of three years within the State of Indiana.

Pay: $18.00 - $20.00 per hour
