Essential Duties and Responsibilities

Makes senior client home visits and completes initial assessments using the appropriate assessment instrument and other required documentation as necessary to enroll the Client for service.

Works with the senior client and/or a personal representative to develop a set of personal goals and a care plan to meet set goals. The Case Manager will arrange, and coordinate, agreed upon services and monitor the quality and effectiveness of the services. Care plan reviews shall be conducted to identify, evaluate, and address the client’s continuing and changing needs.

Arranges for needed services using information care providers such as family and friends as well as formal service providers in the Community Care system. Referrals may be made to Non-Department of Elder Affairs providers for services not available through sub-contracted vendors.

Makes face-to-face visits with the senior client to review the attainment of care plan goals at least every six (6) months. Completes a full assessment of the client’s functioning. Makes telephone contact with each senior client in months not requiring a face-to-face visit.

Documents case activities in an individual confidential case record for each senior client following the Department of Elder Affairs established guidelines.

Completes paperwork, screens waitlists clients for openings as appropriate, helps with requests for information, and makes monthly client contact.

Calls clients and encourages them to prepare for evacuation during an emergency or disaster. Following the event, calls their active caseload to determine the client’s status and assesses needs.

Completes the Department of Elder Affairs assessment training and meets minimum standards for competency testing as required by program standards. In addition, completes a minimum of six (6) hours of in-service training per year. Training and certification on Department of Elder Affairs assessment and care plan form is required. A score of 80% or more is required to maintain employment.

Other duties may be assigned by the Program Director.

Education/Experience Requirements

Bachelor’s Degree in social work, sociology, psychology, nursing, gerontology, or related social services field is required. Year for year job related experience or any combination of education and experience may be substituted for the degree requirement upon approval of the Senior Resource Alliance.

Additional Requirements

Case Manager must have reliable means of transportation, a valid State of Florida Drivers License, and active automobile insurance coverage.

· Must be highly organized, self-motivated, and able to work independently to determine work priorities and meet deadlines.

· Must be able to multi-task in an extremely fast paced environment.

· Must be able to communicate effectively, both verbally and in writing.

· Must present a professional appearance and demeanor to clients, caregivers, coworkers, and Community Partners.

· Must be able to manage and maintain confidential and protected client information.

· Must be mature, honest, and concerned about the welfare of seniors.

Emergency Disaster Assistance

As part of the obligation of Aging Matters in Brevard, being a lead agency serving the elderly, all employees, unless otherwise specified, may be required to be available prior to, during and post disaster. Assignments, as required, will be issued in accordance with directives from Senior Resource Alliance, Brevard County Government and Aging Matters in Brevard Administration.
