HOW YOU'LL MAKE A DIFFERENCE IN THIS ROLE (ESSENTIAL FUNCTIONS)
The Registered Nurse (RN) Resource covers for any nursing needs throughout a specified Agrace team territory. This role is responsible for patient and family satisfaction through the provision of nursing care including but not limited to case management, making assigned patient visits, and conducting admission visits. Responsibilities include comprehensive assessments, following and updating the plan of care, providing interventions, communicating, and documentation.
- Patient and Family Assessments / Case Management
Assess patient and family's physical, pain, psychosocial, emotional, spiritual, environmental and educational needs and update the plan of care (POC).
Respond to patient and family needs, changes in condition, needs for acute patient intervention, and new patient admissions.
Read, understand, update, and follow the POC. Document interventions, outcomes, and changes in patient condition in electronic health record (EHR).
Participate in meetings to plan and evaluate patient / family services.
