I have not felt that anywhere else before" - Agrace Spiritual and Grief Counseling EmployeeHOW YOU'LL MAKE A DIFFERENCE IN THIS ROLE (ESSENTIAL FUNCTIONS)The Spiritual and Grief Counselor serves the interdisciplinary team by responding to referrals, developing and carrying out individualized care plans, and providing individual and group spiritual and grief support.Patient and Family AssessmentsRespond to referrals from clinical teams by contacting the patient and / or family to establish counseling relationship and expectations.Perform spiritual and grief assessment, intervention, and care planning to meet the individual needs of patients and / or families.Engage in ongoing assessment of care plans through interdisciplinary review in accordance with Federal and State GuidelinesWork collaboratively with the patient's designated faith and spiritual community, and refer to other community resources as neededProvide Bereavement SupportDevelop an individualized bereavement plan of care to address the full scope of survivor needs.Provide updates to the interdisciplinary team regarding bereavedConnect bereaved with community resources to assist in the grieving processLead Community Grief Groups and MemorialsDevelop, implement, and facility support groups for Agrace families / community membersProvide supervision and support to counseling services volunteersParticipate in the planning and coordination of Agrace community bereavement events Other DutiesParticipate in internal work groups, meetings and committees relative to position responsibilitiesProvide counseling support (through group sessions, memorial services, reflections, etc.) to the interdisciplinary teams and community partners YOU BRING (EXPERIENCE & QUALIFICATIONS)Master's Degree in Social Work, Counseling (or other related counseling degree), Divinity, Theology, or Pastoral Care, or equivalent combination of education and work experience requiredAt least one unit of Clinical Pastoral Education (CPE) strongly preferredTwo or more years' counseling experience preferredValid driver's license, auto insurance, and access to a reliable vehicleExperience providing counseling and / or spiritual care in both a group and individual settingExperience working with grief as related to deathIntermediate Microsoft Office Suite skillsAbility to work effectively and cooperatively with othersExperience using industry specific softwareAbility to communicate effectively verbally and in writing Ability to be flexible in time and task to meet organizational needsAbility to maintain confidentiality about patients, families, caregivers, staff, donors, and organizational intellectual propertyAbility to work at various Agrace locations Ability to use proper body mechanics and standard precautionsDon't meet these qualifications as written?
