The Spiritual and Grief Counselor works on a collaborative interdisciplinary team including Nurses, Social Workers, CNAs, and Clinical Assistants.

HOW YOU'LL MAKE A DIFFERENCE IN THIS ROLE (ESSENTIAL FUNCTIONS)
The Spiritual and Grief Counselor serves the interdisciplinary team by responding to referrals, developing and carrying out individualized care plans, and providing individual and group spiritual and grief support.
- Patient and Family Assessments
Respond to referrals from clinical teams by contacting the patient and / or family to establish counseling relationship and expectations.
Develop an individualized bereavement plan of care to address the full scope of survivor needs.
Develop, implement, and facility support groups for Agrace families / community members
Participate in internal work groups, meetings and committees relative to position responsibilities
