WHO YOU'LL WORK WITH (AND WHAT THEY HAVE TO SAY)
The Mobile Appointment Assistant supports patients, families, and providers by documenting vitals, concerns, and treatment plans in the patient’s electronic health record (EHR), ensuring fulfillment of agency regulatory requirements.

HOW YOU’LL MAKE A DIFFERENCE IN THIS ROLE (ESSENTIAL FUNCTIONS)
The Mobile Appointment Assistant is responsible for patient and family satisfaction by providing administrative support and in-person customer service for patients during virtual visits with Agrace Physicians and/or Nurse Practitioners (NPs). This position, as directed by Agrace providers, supports patients, families, and providers by documenting vitals, concerns, and treatment plans in the patient’s electronic health record (EHR), ensuring fulfillment of agency regulatory requirements. This role also facilitates technology access and may coordinate interpreter service needs for patient visits, partnering with Agrace clinical staff to deliver efficient and high-quality patient care. The Mobile Appointment Assistant is responsible for patient and family centered care consistent with Agrace philosophy, goals, standards, regulations, and policies.
- Clinical Documentation
- Document patient history, vitals, examination findings, diagnoses, and other changes in patient condition in the EHR, as instructed by the medical provider.
- Document activities related to patient care, including conversations and communications, relevant family information, and provider recommendations in the EHR, as instructed by the medical provider.
- Maintain medical records, including obtaining physical forms and signatures, if required.
- Submit provider orders as instructed to obtain DME supplies, prescriptions, etc. for the patient.
- Provide copies of documentation to patient and/or power of attorney when appropriate, as requested.
- Document and report significant change in condition or unusual incidents to appropriate Interdisciplinary Team Member such as client abuse/neglect, suicidal intent, harmful coping mechanisms or violence, etc.
- Communication
- By virtual resource, connect the patient with the Agrace Physician/NP for interview and exam.
- As directed by Agrace clinical staff, reinforce patient, family, and facility partner education.
- Coordinate communication among nursing staff, physicians, other departments, outside agencies/providers, professional staff, patients, and families as appropriate.
- Facilitate communication through medical interpreter services available, if needed.
- Refer requests that are out-of-scope and related to patient care to the appropriate clinical staff.
- Other Duties
- Participate in internal work groups, meetings and committees relative to position responsibilities

YOU BRING (EXPERIENCE & QUALIFICATIONS)
- High school diploma or GED or equivalent combination of education and work experience
- Valid driver’s license, auto insurance, and access to a reliable vehicle
- Strong verbal and written communication skills; ability to tailor communications to a broad range of audiences
- Ability to work effectively and cooperatively with others
- Ability to use Microsoft Office Suite, messaging and teleconference systems, and virtual meeting software
- Experience with, or ability to quickly learn, industry specific software related to patient medical records
- Ability to be flexible in time and task to meet organizational needs
- Ability to maintain confidentiality about patients, families, caregivers, staff, donors, and organizational intellectual property.
- Ability to use proper body mechanics and standard precautions
