Job Description

The Director of Case Management will oversee the development and implementation of case management programs to ensure high-quality patient care and optimal resource utilization.

Responsibilities

• Program Development and Implementation
• Design, implement, and evaluate case management programs to meet hospital goals and objectives.
• Collaborate with healthcare teams to identify opportunities for process improvement and implement changes.
• Team Leadership
• Lead a team of case managers in developing and implementing care plans that promote efficient use of resources and improve patient outcomes.
• Mentor and coach team members to enhance their skills and knowledge.
• Communication and Collaboration
• Develop and maintain relationships with healthcare providers, payers, and community organizations to ensure effective communication and collaboration.
• Facilitate interdepartmental communication to ensure seamless transitions of care.
• Quality Improvement
• Conduct regular audits to ensure compliance with hospital policies and regulatory requirements.
• Identify areas for quality improvement and develop strategies to address them.
• Financial Management
• Develop and manage budgets for case management programs.
• Analyze financial data to identify trends and opportunities for cost reduction.
• Education and Training
• Provide education and training to case managers on best practices and new technologies.
• Stay up-to-date with industry developments and apply this knowledge to improve case management practices.

Qualifications

• Master's degree in Healthcare Administration or related field.
• Minimum 5 years of experience in case management or a related field.
• Proven leadership and management skills.
• Strong analytical and problem-solving skills.
• Excellent communication and interpersonal skills.
• Ability to work in a fast-paced environment and prioritize multiple tasks.
