The Director of Case Management will oversee the development and implementation of case management programs to ensure high-quality patient care and optimal resource utilization.
Responsibilities
- Program Development and Implementation
- Design, implement, and evaluate case management programs to meet hospital goals and objectives.
- Collaborate with healthcare teams to identify opportunities for process improvement and implement changes.
- Team Leadership
- Lead a team of case managers in developing and implementing care plans that promote efficient use of resources and improve patient outcomes.
- Mentor and coach team members to enhance their skills and knowledge.
- Communication and Collaboration
- Develop and maintain relationships with healthcare providers, payers, and community organizations to ensure effective communication and collaboration.
- Facilitate interdepartmental communication to ensure seamless transitions of care.
- Quality Improvement
- Conduct regular audits to ensure compliance with hospital policies and regulatory requirements.

- Financial Management
- Develop and manage budgets for case management programs.

Qualifications
- Master's degree in Healthcare Administration or related field.
