Job Summary
Provide social work services to patients and families, facilitating their ability to cope with psychological and social problems related to illness and hospitalization.
Responsibilities
- Discharge planning within two working days of admission, involving the patient and family, and relaying information to UR Staff.
- Assisting in determining the presence/type of Advance Directive.
- Reporting suspected cases of abuse/neglect.
- Understanding the role of and accessing the Bioethics Committee.
- Completing initial assessments/care within required time frames.
- Performing subsequent assessments/care at required intervals.
- Incorporating cultural and ethnic factors into assessments/care.
- Differentiating between normal/abnormal findings.
- Facilitating Group Interventions (Group Therapy, Support Groups).
- Coordinating mandated reporting of child abuse/neglect, elder abuse/neglect, and domestic/partner abuse.
- Providing psychotherapeutic intervention to patients/families in crises.
- Arranging for referrals to community services.
- Providing discharge planning.
- Providing in-servicing to staff on social service issues.
- Assisting RNs, physicians, and case managers with resolving domestic issues under a doctor's order.
Requirements
- Masters Degree in Social Work (MSW)
- Experience in medical and psychiatric social work, crisis intervention, and abuse preferred
- Bilingual preferred
- CA License as a Clinical Social Worker preferred
- Current BLS Card
