About the Role
- Collaborate with healthcare professionals to develop and implement patient care plans
- Provide emotional and psychological support to patients and their families
- Conduct thorough assessments and evaluations to identify patient needs
- Prioritize and manage a caseload of patients with complex needs
Requirements
- Master's degree in Social Work from a graduate school accredited by the Council of Social Work
- 1-2 years of postgraduate experience in a healthcare setting
- Demonstrated experience with tablets, mobile phones, and EMR software
