What are the responsibilities and job description for the Care Manager - Critical Time Intervention Program position at AHRC NYC?
AHRC NYC is seeking a Care Manger for their Critical Time Intervention Program (CTI). The CTI Care Manager works to develop trusting and meaningful relationships with people enrolled in the program and develop and maintain relationships with community supports and resources. The Care Manager will assess service needs and contribute to the development of relevant plans make referrals and help people connect with needed supports.

ESSENTIAL RESPONSIBILITIES :
- Works in collaboration with CTI team members to support peoples identified emotional social vocational and housing needs during and in the aftermath of crisis / emergency episodes.
- Provides referral and case coordination services to assist people with achieving stability within their specific environment and capacity.
Utilizing community resources to assist people
