The Recovery Specialist within ACG Health Recovery will serve as a motivating support person and role model for patients attempting to lead a life of recovery. The individual will set a positive example that instills hope for a better future and connect patients to recovery resources. In addition, they will provide comprehensive, community-based care management for patients in the Center of Excellence program.

Responsibilities :

  • Greet and acknowledge all patients according to agency standards
  • Receive, record and relay messages accurately, completely and legibly
  • Speak with others using clear, professional and polite language
  • Develop and maintain positive working relationships with others, support the ACG team to reach common goal
  • Care Manager responsibilities :

  • Serve as a resource for all patients of CIM living with opiate use disorder with varying degrees of need
  • Conduct intakes following program guidelines
  • Use standardized tools to conduct initial suicide screenings and connect patients with internal mental health resources if they screen positive, following program protocol
  • Create and update initial and subsequent care management plans
  • Meet patients as necessary depending on their need, at least once per month
  • Ensure that all patients maintain active insurance
  • Reach out to patients who miss appointments
  • Outreach to patients who have left the program to provide support and demonstrate an open door for return to the program.
  • Refer patients and track referrals to specialists, treatment facilities, community resources, etc.
  • Comprehensively evaluate patient needs and social determinants of health (using a standardized tool) and work as part of the treatment team to advocate for and address needs
  • Help patients with identification, navigating systems, benefits, housing, food insecurity, transportation, etc.
  • Connect patients to internal care navigators as necessary
  • Attend treatment team case conferences twice each week, participate actively in treatment team discussions, and discuss your assigned patients as needed
  • Document all patient interactions in the electronic health record
  • Attend required trainings, including those offered in-house, and those offered by the PA Center of Excellence program
  • Encourage patients to participate in therapeutic and social activities that enhance recovery skills and develop "sober" relationships
  • Collaborate with medical staff, receptionists, therapists, and medical providers to work with people living with addiction and mental health struggles to engage in treatment and maintain themselves in the community
  • Attend required monthly state-wide COE Peer Support meetings
  • Collaborate with other CRSs at the agency
  • May be ask to participate in outreach efforts through other departments
  • Build and maintain relationships with outside programs to facilitate patient admission as needed
  • Work with patients to enter higher levels of care as needed
  • Actively monitor the program’s Teams communication channels and respond in Teams as needed
  • Other duties as assigned
  • Requirements :

  • PA Certified Recovery Specialist credential
  • Previous experience working as a CRS required
  • Knowledge of working within a COE strongly preferred
  • Ability to demonstrate competency in personal recovery and be a role model for patients
  • Ability to work in an electronic health record, such as eClinicalWorks
  • Ability to forge strong relationships with patients living with opiate use disorder and other mental illnesses
  • Previous exposure to motivational interviewing
  • Ability to effectively manage time and prioritize responsibilities
  • Ability to work effectively within teams
  • Must have strong attention to detail
  • Knowledge of the Delaware County, PA, region and / or a willingness and ability to learn about the region
  • Good interpersonal skills and the ability to work independently and as a member of a team

  • Proficiency with MS Office
  • Must have reliable transportation and the ability to travel from site to site as needed
