Education/Experience:
An associate or bachelor’s degree in psychology, sociology, human services, social work or a related field is desirable. Experience working with unsheltered individuals for housing preferred. Previous experience in working with individuals for skill building, life skill training, job seeking preferred.

General Tasks:
· Develop a plan of outreach into the community that will provide for consistent contact with unsheltered individuals through social service programs, the Continuum of Care (CoC) or their places of habitation.
· Develop a network of unsheltered assistance and social service providers. Partnering with agencies to bring skill building services to the clients.
· Identify individuals that are unsheltered that may be appropriate for ASC Home ARP program from the COC admission referrals committee.
· Identify apartment, or housing opportunities to be leased for individuals coming into the programs, as well as conducting necessary assessments.
· Develop and monitor individual service plans and housing plans.
· Assess needs and resources of other unsheltered individuals during outreach work and assist them in accessing other forms of aid for which they may qualify.
· Work in cooperation with other caseworkers on staff at ASC to provide case conferencing, assistance, and resources.
· Enter demographic information, assessments, complete contact log information, and upload supporting documentation to the Homeless Management Information System (HMIS) with efficiency and timeliness following a client contact.
· Maintain timely and detailed statistics and client case records as required by ASC and HUD and generate program reports as requested including necessary quarterly and annual reports.
· Attending Continuum of Care (CoC) meetings at Southeast Homeless Coalition.
· Other tasks as assigned by supervisor.
