Shifts:
- Full-Time, Days
Key Responsibilities:
- Assist in coordinating and implementing social services programs for residents
- Support residents and families in adjusting to facility life and addressing concerns
- Help develop and maintain care plans that align with resident needs and preferences
- Facilitate resident and family meetings to provide guidance and assistance
- Provide resources for financial, emotional, and legal support as needed
- Assist with discharge planning and ensure a smooth transition for residents returning home or to another care setting
- Maintain accurate documentation and reports in compliance with facility policies and regulations
Qualifications:
- Experience in social services, preferably in a healthcare or long-term care setting
- Strong understanding of resident rights and care planning processes
- Excellent communication and interpersonal skills
- Ability to work compassionately with residents, families, and interdisciplinary teams
- Associate's or Bachelor's degree in Social Work or a related field is preferred
