Position Snapshot : The CDPAP Facilitator maintains a minimum caseload of 200 participants who are individuals with disabilities and their caregivers to successfully meet the requirements associated with CDPAP. This can include remaining independent in the community by arranging and administering consumer-directed care.

- Provide new and existing consumers with transitional services.
- Ensure time and attendance compliance by monitoring usage and providing education where needed.
- Return all consumer calls within 24 hours, and promptly address all questions and concerns as needed.
