We are looking for a skilled and compassionate Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our telehealth team and deliver high-quality care to our patients.
Job Description : As a Telehealth PMHNP, you will be responsible for conducting virtual consultations, assessments, and follow-up appointments with patients. You will work collaboratively with our mental health team to develop and implement personalized treatment plans.
Responsibilities :
- Conduct psychiatric evaluations and assessments
- Develop and implement individualized treatment plans
- Prescribe medications as appropriate
- Monitor patient progress and adjust treatments as necessary
- Maintain accurate and timely patient records
- Provide patient education and support
- Collaborate with a multidisciplinary team to ensure comprehensive patient care
