The Women's Health Team at Aizer Health is a dynamic and compassionate team dedicated to delivering exceptional care to our patients. As a Registered Nurse, you will play a crucial role in providing professional nursing care to patients and their families, primarily via telephone communication, following established standards and practices.
Key Responsibilities
You will be responsible for:
- Coordinating with other members of the Women's Health Team to ensure seamless care delivery.
- Performing telephone triage and educating patients about their healthcare.
- FOLLOWING UP WITH PATIENTS NEWLY DIAGNOSED WITH CHRONIC DISEASES, DISCUSSING LAB RESULTS, MEDICATIONS, AND ADDITIONAL SUPPORT NEEDS.
- Conducting Prescription Monitoring Program checks for all controlled medications.
- PERFORMING CARDIAC STRESS TESTS AND PROVIDING RESULTS TO THE WOMEN'S HEALTH TEAM.
- Assisting providers with patient examinations and procedures as needed, including administering medications, taking vital signs, and immunizations.
- DOCUMENTING ALL CARE PROVIDED IN EClinicalWorks (EMR).
- Adhering to Aizer Health's medical protocols, policies, and procedures.
- Acting as a patient advocate and ensuring compliance with HIPAA regulations.
Requirements
To be successful in this role, you should have:
- A graduate degree from an accredited school of nursing and a current RN license in New York State.
- CPR certification required.
- Phlebotomy experience is a plus.
- Electronic Medical Records (EMR) experience preferred.
- Strong commitment to teamwork and the ability to take initiative.
- Excellent communication and interpersonal skills with the ability to relate effectively to medical staff and patients.
