About the Job
The Registered Nurse will work closely with the Women's Health Team to provide professional nursing care to patients and their families, primarily via telephone communication, following established standards and practices. Key responsibilities include:
- Coordinating with other members of the Women's Health Team to ensure seamless care delivery.
- Performing telephone triage and educating patients about their healthcare.
- FOLLOWING UP WITH PATIENTS NEWLY DIAGNOSED WITH CHRONIC DISEASES, DISCUSSING LAB RESULTS, MEDICATIONS, AND ADDITIONAL SUPPORT NEEDS.
- Conducting Prescription Monitoring Program checks for all controlled medications.
- PERFORMING CARDIAC STRESS TESTS AND PROVIDING RESULTS TO THE WOMEN'S HEALTH TEAM.
- Assisting providers with patient examinations and procedures as needed, including administering medications, taking vital signs, and immunizations.
- DOCUMENTING ALL CARE PROVIDED IN EClinicalWorks (EMR).
- Adhering to Aizer Health's medical protocols, policies, and procedures.
- Acting as a patient advocate and ensuring compliance with HIPAA regulations.
