SUMMARY OF RESPONSIBILITIES

The Community Programs (CP) Case Manager is responsible for the provision of Intensive Case Management; Case Management; and therapeutic skill building services within the Therapeutic Treatment Home and Home Based Services programs.  Duties include crisis intervention, advocacy, assessment, referrals to necessary services, discharge planning, treatment planning, individual; family; and group skill development services, supporting youth access to services / transportation, interdisciplinary teamwork and community liaison.

The CP Case managers are responsible for meeting agency productivity standards by implementing appropriate treatment strategies, submitting timely and satisfactory documentation, under the supervision of the Clinical Case Manager Supervisor. The CP Case Manager participates in a rotating on-call service and is responsible for these duties as outlined in policy when on-call.

CP Case Managers are supervised by the Clinical Case Manager Supervisor, and participate in supervisory consultation at least weekly. They are expected to meet at least weekly with each youth on their assigned caseload, in addition to completing more routine case management tasks on behalf of youth on their caseload, to adhere to best practice.

Adherence to the standards and regulations established by the Agency, OCS Licensing, Division of Behavioral Health, The Joint Commission, and Medicaid is mandatory. The responsibilities of the Community Programs Case Manager will be carried out in accordance with the policies of AK Child & Family, as well as the organization's mission, core values, and care philosophy. Additionally, the Community Programs Case Manager may assume further duties as assigned.

  • 23.00 / hour depending on experience

  • Knowledge, Skills and Abilities :

  • The CP Case Manager must have the ability to relate to and experience in working with children / adolescents who have experienced trauma and may become deregulated and their families.
  • Demonstrated ability to complete assessments, treatment planning, parent training, advocacy skills and case management skills are also required.  The ability to provide written and oral communication in English, along with demonstrated interpersonal / social skills is essential.
  • Creativity / flexibility must be demonstrated in the delivery of services.
  • Excellent time management, organizational skills, and computer skills (MS Office Suite, telehealth and electronic health records) including timely completion of treatment notes are also necessary qualifications.
  • The CP Case Managers must demonstrate the ability to function as a member of the interdisciplinary treatment team, and display an understanding and sensitivity to serving a culturally diverse and / or special needs population.
  • Education and experience :

  • The CP Case Manager is required to possess a Bachelor’s Degree in psychology, counseling, social work, or a closely related discipline such as studies in child development or human services.
  • Must have one-year experience working with families, and or children / adolescents who have lived or experienced trauma, or emotional dysregulation.
  • Substitution for education requirement will be given for year for year for candidates who have experience working with these demographics.
  • Preference will be given to Candidates with experience in case management, as well as knowledge of psycho-educational skill development, cognitive and reality-based therapies.
  • Physical Demands :

  • The Community Programs Case Manager must be able to access AK Child & Family and community buildings for meetings and agency activities.
  • The CP Case Managers is required to have a valid driver’s license and an adequately insured personal vehicle for reliably getting to and from work responsibilities as work responsibilities require.
  • They will transport self and others as needed to meet with schools, families and other involved parties.
  • They must produce in a timely manner, assessments and other documentation required by AK Child & Family, insurance and referral sources.
  • The Community Programs Case Manager must be able to navigate internet-based systems, electronic health record, and must be able to use a copy machine, fax machine, as well as to produce thorough youth records and prepare reports using a computer.
  • Supervision & Coordination of Services :

  • The Clinical Case Manager Supervisor supervises CP Case Managers.
  • The CP Case Manager coordinates services with CP Behavioral Health Specialists, Therapeutic Treatment Parents, and members of the Treatment Team as appropriate, in consultation with the Directing Clinician and Clinical Case Manager Supervisor.
