Medical Social Worker
Location: Syracuse, NY 13210
Duration: 13 weeks
Shift: 8 hour day shift
Pay rate: $62/hr. on w2 with benefits
Weekly expected hours: 40
Preferred Qualifications: 3 years in a hospital and/or healthcare setting.
The social worker will be assigned to provide coverage as needed at any of our facilities. The social worker provides comprehensive psycho-social assessment, intervention, management, support, and assess for discharge planning and aftercare needs of the at-risk patient population and theirfamilies. In addition, the social worker addresses social determinants of health and works collaboratively with the multidisciplinary team. Other assigned duties and tasks based upon operational needs.
